Chung Kuo The Middle Kingdom by David Wingrove

Read this back in college (blame Knepper ;); it paints a future where the world has much more population, countries have expanded into mass city-states (Europe, US, South America, Asia, each is one large city) built into strictly controlled layers where the further from the earth you were, the better your social standing. The apex of this society lives on an orbital station.
